Our Services
Patients become a part of this practice by signing up for a monthly plan. There are no charges for other services including office visits, in office testing and procedures. Monthly fees are based on age.
Insurance is not billed under this model of care.

Includes: Comprehensive Primary Care with unlimited visits, Annual Physical with Basic Labs Included, Direct texting via patient portal, Same day or Next Day availability, Additional Labs/Imaging offered at reduced prices, Prescription management, Chronic disease management, Coordination with specialists/referrals.

Ages 27 & Up
$109/Mo.
Includes: Comprehensive Primary Care with unlimited visits, Annual Physical with Basic Labs Included, Direct texting via patient portal, Same day or Next Day availability, Additional Labs/Imaging offered at reduced prices, Prescription management, Chronic disease management, Coordination with specialists/referrals.
Patient visits and billing begins on September 4th, 2026
Enrollment Fee: One-time non-refundable enrollment fee of $49 per person. Includes: obtaining prior medical records, comprehensive initial physical exam, and onboarding process.
